READY TO REFER?

If you are ready to refer a patient, you may fax orders to (210) 500-2422. You may simply write an order for "neuropsychological evaluation" and address the most reasonable medical condition or process that would relate to the presentation (e.g. "memory loss, please assess for possible dementia", or "CVA, assess for possible return to work", etc.). Including an ICD-10 diagnosis will assist with possible insurance reimbursement. You may also use our "Physician's Referral for Neuropsychological Evaluation" form, which allows for easy check-boxes to indicate what you need from an evaluation (call our offices and we will send you a copy, 219-243-1912). 

We would be happy to discuss any questions you may have and provide brief phone consultation about specific referrals as needed.

THINKING ABOUT REFERRING?

If you are a physician considering referring one of your patients, we look forward to being of service.

For patients experiencing concerns with thinking abilities or behavior related to brain functioning (e.g. suspected dementia, late effects of head injury, stroke, or any other condition that may impact cognition), neuropsychological evaluation can address specific diagnostic concerns, as well as practical considerations, such as the following:

  • Is this dementia or normal aging?
  • Should this patient be managing her finances independently? 
  • Is my patient really ready to return to work after his stroke?
  • Should this college student return to classes this coming semester following her head injury?

Neuropsychological evaluation can also provide specific recommendations for both the patient and family for how to live with these challenges in the most positive and effective way possible, depending on their unique challenges and resources. Ongoing consultation is also provided to patients for a limited time following evaluation, to fully address their concerns.
